Reports to: Director of CalAIM
Status: Full-Time
Classification: Non-Exempt (Eligible for overtime in accordance with California law)

Location: Fresno Office (Community-Based Field Work Throughout Fresno County and Surrounding Areas)
Schedule: MON-FRI, 9:00 AM – 5:30 PM
Compensation: $23-$25/HOUR

Position Overview
MISI Community Solutions is seeking a mission-driven, relationship-oriented Community Partnerships Lead to help expand access to vital health and social support services for Medi-Cal members through California’s CalAIM program in Fresno and surrounding communities.

This role focuses on building trusted partnerships with community organizations, healthcare providers, shelters, schools, and other referral sources to identify and connect eligible Medi-Cal members with MISI services. This is a consultative, relationship-based position centered on long-term community engagement and community trust.

Performance-based incentive compensation may be available and, if applicable, will be governed exclusively by a separate written agreement in accordance with California law.

ABOUT THE ROLE
Outreach Specialists build trusted community partnerships and connect eligible Medi-Cal members to CalAIM Community Supports services. You’ll conduct outreach, generate referrals, and support member enrollment through relationship-driven engagement throughout Fresno County.

Key Responsibilities
a. Develop and maintain relationships with community partners throughout Fresno County and surrounding areas
b. Identify and engage eligible Medi-Cal members through trusted referral channels
c. Conduct outreach meetings, presentations, and community engagement activities
d. Guide prospective members and partners through referral and enrollment pathways
e. Maintain accurate records of outreach activity, referrals, and partner engagement
f. Collaborate with Intake, Care Coordination, and Operations teams to support successful enrollment and service delivery
g. Serve as a professional ambassador representing MISI in the Fresno community
h. Participate in ongoing training, coaching, and professional development

Required Qualifications
a. Fluency in English and Spanish (spoken and written)
b. Strong interpersonal and relationship-building skills, coachable team player
c. Excellent communication and presentation abilities
d. Highly organized with strong attention to detail
e. Self-motivated with the ability to work independently and as part of a team
f. Comfortable working in community settings and conducting outreach
g. Reliable transportation and ability to travel locally within Fresno County

Preferred Qualifications:
