Enhanced Care Management is an essential Medi-Cal benefit available statewide, designed to support individuals with significant medical, mental health, or social needs. Through ECM, participants work with a dedicated Lead Care Manager who coordinates a network of health care and social services. This program is especially valuable for individuals living with Serious Mental Illness (SMI) or Substance Use Disorder (SUD).

MISI’s Enhanced Care Management program ensures that each individual receives personalized, holistic support. We help clients navigate complex healthcare systems, connect them with resources, and provide ongoing guidance to improve health and stability.

Case Study - Faces of Medi-Cal’s Transformation: Meet Frank

Frank faced significant challenges with opioid addiction and lived unhoused on the streets of San Francisco for four years. Over two years, he visited emergency departments seven times due to overdoses and returned to the streets after short shelter stays. In 2020, Frank contracted COVID-19 and continues to deal with lasting symptoms. During an emergency department visit, he was referred to MISI’s Enhanced Care Management program. His Lead Care Manager met him regularly at a local food bank to build trust and help coordinate his care. Together, they developed a plan for Frank to reconnect with his mental health provider, adjust his medications, and maintain follow-up visits with his primary care doctor. The Lead Care Manager also connected Frank to Community Support services, ultimately helping him secure safe, stable housing.
