MISI Expands ECM Services Through New Collaboration with Niazi Family Medicine
Sacramento, CA – July 2025 – My ISI (MISI) is proud to announce a new collaboration with Niazi Family Medicine aimed at expanding access to Enhanced Care Management (ECM) services for patients in the community.
During a recent meeting, representatives from MISI and Niazi Family Medicine discussed ways to work together to better serve individuals with chronic health conditions, those experiencing or at risk of homelessness, and patients with complex care needs. The meeting concluded with a mutual commitment to refer eligible patients from Niazi Family Medicine to MISI’s ECM program, ensuring they receive coordinated, compassionate, and culturally sensitive care.

Said MISI’s Marketing Manager, Hafiz Mohammadi, who attended the meeting on behalf of MISI. “By working closely with trusted healthcare providers like Niazi Family Medicine, we can reach more patients who need our support and connect them to the resources that make a real difference in their lives.”
Through ECM, MISI provides once-in-a-lifetime services such as housing navigation, medically supportive food, care coordination, and more tailored to each individual’s needs. With this new collaboration, patients of Niazi Family Medicine will have easier access to these services, ultimately helping improve their quality of life and reduce barriers to care.
